Health Care Center

Director of Nursing

Primarily day shift but flexibility with hours is required.

General Job Duties: Plan, organize and develop and direct the overall operation of the nursing department in accordance with federal, state and local regulations. Ensure care and services provided to elders, are person-centered and in accordance with accepted standards of nursing practices and facility policy. Exhibit and model the principals of person-centered care in all interactions.

Monitor quality indicators reports to identify potential areas of concern and care data overall. Ensure that corrective actions are taken to address areas of concern. Keep up to date on current federal and state regulations, as well as professional standards and make recommendations on changes to policies and procedures to the VP of Health Services.

Job Requirements: Minimum of 3 years previous experience in a management role in nursing services. Working knowledge of Federal and PA State regulations governing long term care. Excellent leadership, communications and organizational skills. Ability to travel, as needed to represent LV at conferences and industry meetings.

RN Case Manager/Admissions

Full Time: 9A-5P with some flexibility required

Coordinates overall admissions process into the Health Care Center. Assesses potential admissions in their home or acute care center for appropriateness. Acts as the primary liaison/contact with managed care insurances. Tracks, evaluates and reports on all 30 day re-admissions and develops QAPI’s on problem areas. Understands and explains to families the admission criteria for the different levels of care including review of applications for financial feasibility.

Current RN license of the State of Pennsylvania

Must have valid driver’s license

Must be an excellent communicator

RN Assessment Coordinator

Full Time

Duties will include completion of the MDS in compliance with State and Federal Guidelines.

Requirements include current Pennsylvania RN license, extensive knowledge of MDS, and Care Plan process, working knowledge of LTC and Medicare regulations, and proficiency with computer programs. The ability to work cooperatively with all members of the resident care team is essential. Two years of experience as an RNAC preferred. We will also consider LPN’s who have at least two years of experience as an LPNAC.

RN Nurse Manager

Full Time with $5,000 sign on bonus: Responsibilities include monitoring care and services provided to our residents to ensure a focus on the individual. The Nurse Manager will also be responsible for staff supervision and serving as a clinical resource for staff. Excellent nursing assessment and organizational skills a must.

Floor Care Specialist

Full Time

  • Collects necessary equipment and supplies and proceeds to assigned work area
  • Has general knowledge of housekeeping equipment and its use and notifies supervisor of equipment needing repair
  • Practices established infection control procedures and wears appropriate personal protective equipment
  • Adheres to all safety rules and reports safety hazards to management
  • Returns unused unsoiled supplies to appropriate area at the end of each work day
  • Keeps carts, closets and equipment neat, clean and orderly at all times
  • Uses proper procedure, chemical (mixtures) and equipment for each task
  • Cleans equipment before returning it to storage area
  • Maintains equipment in working order up to and including basic preventative care
  • Uses the WorxHub Software for billing, scheduling, and Quality Reporting
  • Understands and completes housekeeping and laundry tasks as assigned by supervisor or team leader when necessary
  • Moves equipment and/or furniture for relocation of rooms or offices
  • Shampoo carpets, furniture and mats, as instructed
  • Cleans resident rooms, bathrooms, offices, halls and other areas
  • Assists in moving furniture and/or rearrangements of furniture
  • Delivers and collects linens as assigned
  • Operates floor equipment and ensures that floors are maintained daily
  • Disinfects floor care equipment after each use
  • Cleans baseboards throughout the facility
  • Responds to floor care emergency calls; such as, spills, bodily fluid spills, and markings, using proper equipment to clean area
  • Other duties as assigned
