At Londonderry Village, nursing care is thoughtfully designed to meet residents where they are—whether recovering from an illness or injury, or in need of long-term support. Care is provided in two distinct settings: the Leffler Transitional Care for short-term rehabilitation and transitional care, and our Small Homes for long-term nursing care.

Leffler Transitional Care
Based on physician referral, our rehabilitation team partners with each patient to achieve personalized recovery goals through inpatient therapy in the Leffler Care Center. This setting is designed for short-term, goal-driven care following a hospital stay or medical event.

Our experienced clinicians provide physical, occupational, and speech therapy for a wide range of conditions, including stroke, heart conditions, joint replacements, and neurological or orthopedic disorders. Patients benefit from access to state-of-the-art therapy gyms and fully equipped exercise spaces to support optimal recovery.

In addition to serving Londonderry Village residents, rehabilitation services are also available to members of the surrounding community. Every care plan is focused on helping individuals regain strength, independence, and function at the highest level possible.

Long-Term Care in Our Small Homes
For those in need of ongoing nursing support, Londonderry Village offers a more intimate and personalized approach through our Small Homes.

Each 7,000-square-foot home is designed for just ten residents, creating a calm, familiar environment that feels more like a home than a traditional care setting. Residents enjoy private bedrooms, shared living and dining spaces, and an open kitchen where home-cooked meals are prepared daily.

Care is provided 24/7 by specially trained Certified Nursing Assistants, supported by licensed nurses who are always available. Team members assist not only with medical needs, but also with meals, activities, and housekeeping—fostering meaningful relationships and a strong sense of community.

This model supports dignity, independence, and choice, allowing residents to maintain their routines and live life on their own terms while receiving high-quality, personalized care.
