That belief is at the heart of why we’ve joined the Age-Friendly Health Systems movement — a national initiative designed to improve health outcomes and quality of life for older adults through the proven 4Ms framework.
Age-Friendly Health Care: What Patients Need and Deserve
As older adults live longer, more active lives, our health care systems must evolve to support them in ways that reflect their values and needs. Age-Friendly Health Systems — an initiative led by The John A. Hartford Foundation and the Institute for Healthcare Improvement — aims to deliver reliable, respectful, and high-quality care for every older adult, in every setting.
By participating in this initiative, Londonderry Village is part of a growing movement of hospitals, nursing homes, and senior communities across the country working to align care with what matters most to older adults. Being an Age-Friendly Health System means we’ve committed to implementing evidence-based practices that ensure safety, dignity, and independence — while also striving to reduce avoidable hospitalizations, lower costs, and improve satisfaction for both residents and families.
The 4Ms Framework: Guiding Our Care
As a proud participant in the Age-Friendly Health Systems Action Community, Londonderry Village is implementing the 4Ms — four essential elements that define age-friendly care:

What Matters: We listen to and prioritize each resident’s goals, values, and preferences in their care plans.

Medication: We use only necessary medications that support, rather than hinder, cognitive function, mobility, and personal goals.

Mentation: We actively prevent, identify, and manage conditions like dementia, depression, and delirium.

Mobility: We promote daily movement and physical activity to help residents maintain function, independence, and quality of life.

What Matters & Mentation: Barteit Haven Memory Care Neighborhood
Barteit Haven, our secure and supportive memory care neighborhood, offers a deeply personalized approach to dementia care. We begin by learning each resident’s life story — the people, routines, and moments that shaped them. From that foundation, we create daily experiences centered around What Matters, including meaningful activities like music therapy, interactive storytelling, and sensory engagement. By fostering familiarity and connection, we actively support Mentation and help residents feel grounded and understood.
Medication & Mobility: Green Houses Skilled Nursing
Our Green Houses reimagine traditional nursing care in a warm, small-home environment. Each resident has a private room and is supported by consistent caregivers who get to know them well. Medications are administered carefully to avoid those that could interfere with cognition or physical ability. We encourage daily movement — whether it’s walking to a family-style meal, enjoying the outdoor patio, or helping with household tasks — so residents maintain Mobility and continue doing what brings them joy and purpose.

We are actively testing, refining, and sustaining these practices across campus to ensure that every resident receives care that is proactive, personal, and empowering.
